This soul shape dropped in Torghast of the Damned - Skoldus Hall Layer 10 Floor 2. This soul shape can drop on any floor; this is just the one it dropped on for me.

This soul will be found as a chained Lost Soul; all you need to do is free the Lost Soul by clicking it. After you free it, the quest will pop up. Just accept it and turn it into Lady Muunn.
